SUN-102 - Usability of FEMPOWER: A Novel Fertility Education Tool for Female Adolescent and Young Adult Cancer/BMT Survivors

Background
Survival rates for pediatric cancer and bone marrow transplantation (BMT) continue to increase, underscoring the importance of addressing late effects from treatment such as infertility. Fertility impairment and fertility-related uncertainty are associated with psychosocial distress and decreased quality of life. Despite current guideline recommendations for reproductive health assessments and fertility counseling, knowledge and practice gaps remain. Thus, our team has developed FEM-POWER (Fertility Education Module: Puberty, Ovulation, Wellness, Reproductive Health), which is a web-based, patient-facing education tool designed to improve awareness, facilitate clinician–patient–caregiver discussions, and prevent missed opportunities for referrals and clinical interventions during adolescence and young adulthood. The goal of this study was to obtain patient, caregiver, and clinician perspectives on the content and format of FEM-POWER.

Methods
FEM-POWER was developed by a multi-disciplinary team of specialty physicians, clinical researchers, and digital health specialists. We recruited female adolescent and young adult survivors of cancer/BMT, caregivers and clinicians to participate in usability testing and semi-structured interviews (patients/caregivers in dyads). The interview guide asked about overall acceptability, navigation capacity, clinical utility, and feedback for improvements. Interviews were audio recorded and transcribed. The codebook initially contained deductive codes but was iteratively refined with inductive codes emerging from the data. Thematic analysis was conducted by three independent coders working in rotating pairs. The third coders served as a neutral party to resolve discrepancies. From this process, the codebook was refined, data were analyzed, and final themes were determined.

Results
This was a single center study that included female survivors, (n=10), caregivers (n=9) and clinicians (n=7). The age range of the patients was 10-22 years old, and they had a variety of cancer and BMT-related diagnoses. Analysis of the participant interviews revealed three themes: 1) FEM-POWER was generally liked by all three participant groups and intuitive to use, 2) FEM-POWER simplified topics and enhanced education that had been provided during oncofertility counseling and 3) modifications for improvements on usability and content were suggested (e.g. adding information about fertility preservation options and suggested questions for the healthcare team).

Conclusions
Our data conclude that FEM-POWER had an overall positive perceived value in learning key reproductive health information and was easy to use. The data from this study will directly inform FEM-POWER revisions, after which feasibility and acceptability will be tested in the clinical setting. Future research will evaluate efficacy and implementation.
