Learning Objectives:

  • Diagnose and differentiate premenopause, perimenopause, and menopause in midlife women presenting with weight gain or central adiposity by using clinical features and appropriate hormonal evaluation.
  • Differentiate the causes of obesity in menopausal women, including menopause-related physiologic changes in body composition, endocrine disorders, lifestyle contributors, and central fat redistribution, using waist-based metrics alongside body mass index (BMI).
  • Explain the metabolic effects of menopausal hormone therapy (MHT) and emerging alternatives, including their influence on fat distribution, and insulin sensitivity and their potential integration with lifestyle and pharmacologic treatments. Furthermore, recognize non-hormonal modifiers of metabolic risk during menopause, including diet quality, energy expenditure, sleep disruption, and adipokine involvement (eg, leptin).
