MON-580 - Impact of Patient Age on Continuous Glucose Monitoring Prescription Patterns in A Safety‑Net Hospital Settings

Introduction:
The use of continuous glucose monitoring (CGMs) has changed diabetes care over the past decade. As studies show improved outcomes for patients with diabetes who have access to CGMs, encouraging the use of CGMs in patients who have yet to be introduced to this technology is increasingly important.

Methods:
Surveys recording patient willingness to adopt the use of CGMs and any barriers to receiving a CGM were administered to 545 patients in the diabetes clinic of a safety-net hospital. We analyzed these survey responses and conducted chart reviews for the corresponding patient over the course of approximately one year (March 5th, 2024 to January 31th, 2025). Whether each patient received a CGM prescription after completing the survey was recorded. Patients were then categorized into two groups: those who had a CGM order placed (yes order) and those who did not (no order).

Results:
Among 545 unique patients, 222 expressed interests in receiving a CGM prescription; however, only 158 ultimately had an order placed by their provider, and only 26 (16%) were confirmed to be active CGM users based on LibreView® and Dexcom Clarity® data within one year of order placement. There were no significant differences in sex, race, or diabetes type between patients who received CGM prescriptions and those who did not. However, patients who received prescriptions were significantly younger than those who did not (53.02±13.69 vs 57.13±13.40 years, p=0.002). Patients who were prescribed CGMs also had a slightly higher Hb A1C than those who were not (9.01±2.53 vs 8.41±2.28, p=0.0002). Additionally, patients in the no-order group were more likely to report inability to afford the $75 copay card prescription cost.

Conclusion:
Despite broader insurance coverage for CGM, substantial systemic, provider level, and patient level barriers continue to limit uptake, with only 16% of eligible individuals ultimately using CGM. Older patients were less likely to receive CGM prescriptions, highlighting the need for further investigation into the factors driving this disparity and for targeted strategies to support CGM adoption among older patients living with diabetes. Further investigation into the reasons patients who received prescriptions for CGMs did not ultimately use them must also be done to address other gaps of care.
